List the purpose, previous diagnosis or referral, report dates, medicines and allergies. A summary helps communication but does not replace original records.
Language & accompaniment
State your main language, dialect needs and department. Ask about relevant experience, confidentiality and support for complex discussions. A translated webpage does not establish interpreter availability.
